The following documents must be attached with the application form attested by a gazetted officer of the 1st class (must bear the seal bearing the name of the attesting officer) – a) 03 (three) copies of passport size color photographs taken recently. 02 (two) copies of 03 copies of photographs should be attached to the application form and 01 (one) copy to the admit card. b) All original/temporary certificates of educational qualification and certificates of training issued by the Board/University; c) Citizenship certificate issued by Union Parishad Chairman/Municipal Mayor.
